This is a youth soccer registration form. By signing this form, you and your child agree to follow the team’s rules and expectations. This registration allows your child to participate in team practices, games, and events. If the rules or expectations are not followed, the club may end the membership at any time

Players Expectations

1. Commitment to the Soccer Program

We understand that being part of a team means showing up and doing our best.
Players are expected to attend practices, games, and team events regularly. If a player can’t make it to a session, parents should let the coach or team manager know ahead of time.

2. Commitment to the Club Mission

Our mission is to help kids learn soccer, build character, and grow as teammates and individuals.
We aim to provide a fun, safe, and positive environment where every player can develop their skills and love for the game.

3. Family Involvement & Volunteering

To make our program successful, we may ask families to help with things like team snacks, setting up for games, team events, or fundraisers.
Volunteering is a great way to support your child and the team.

5. Player Expectations & Code of Conduct

As a player, I agree to:
a. Arrive on time for practices and games, and let my coach know if I can’t make it.
b. Listen to my coaches and respect their decisions.
c. Be kind and supportive to my teammates on and off the field.
d. Show good sportsmanship — win or lose.
e. Represent my team with pride and respect.
